Dear Patient, As many of you know, I have operated two locations since January 2011, and have been searching for a permanent location for the McKinney office, Fairview Family Medicine.  Unfortunately, this is taking longer than expected.  This impacted our plans to relocate my Dallas office, Lakeside Family Medicine, to the Forest Park Medical Center campus at the end of last year.

Beginning Monday, 23 January 2012, the McKinney office will temporarily relocate to the Forest Park Pavilion building, 12222 North Central Expressway, Suite 200, Dallas, Texas 75243 (on the north side of the Forest Park Medical Center campus), at the northeast corner of Forest Lane and North Central Expressway.  This site will be known as Forest Park Family Medicine.  The office will be located in a new and beautiful medical office building with many amenities.  Our days in the office, and hours of operation will be: Tuesdays, and Fridays, from 9:00am to 12:30pm, and 1:40pm to 5:00pm.  I will continue to see patients at Lakeside Family Medicine on Mondays, Wednesdays, and Thursdays from 9:00 am to 12:30pm and 1:40pm to 5:00pm.  Once permanent space for the McKinney office is located and a lease is signed, the Forest Park location will become my only office in Dallas.
