Patients may new access the online patient portal to view medical records, request prescription refills, make appointments and update contact and insurance information. We also added a new feature which allows patients to make payments through our payment processor Phreesia.

Dear Patient,
My Garland Road office, Lakeside Family
Medicine, has closed permanently. While medical records for patients seen at
this office have been transferred to my North Dallas office, Forest Park Family Medicine, patients may be seen at either office location.
Forest Park Family Medicine located at 12222
N. Central
Expy., Suite 200, Dallas, Texas 75243 (in the Forest Park Pavilion, on
the
north side of the Forest Park Medical Center campus), at the northeast
corner
of Forest Lane and North Central Expressway. I continue to see patients
there on Tuesdays and Fridays, from 8:30am to
12:30pm and from 1:40pm to 5:30pm. The
office will be closed for lunch from 12:30pm to 1:40pm.
My Collin County office, Raintree Family Medicine, is now open, and is located in
suite 180 of the Allen Medical Pavilion, at 997 Raintree Circle, Allen, Texas
75013, (on the west side of N. Central Expressway, between Watters Road and
Allen Street). Office hours will be
Monday and Thursday from 9:30am to 12:30pm and from 1:40pm to 4:30pm, and on
Wednesday from 9:30am to 12:30pm. The
office will be closed for lunch from 12:30pm to 1:40pm. This is a brand new office building, and the
suite has been built out to my specifications, allowing me to offer my patients
more and better services in the coming years.
I will have expanded diagnostic radiology capabilities, as an imaging
center is co-located in the building.
CPL and Quest laboratory drawing stations are located within two blocks
of the office.
